مقدمه: در سال­های اخیر، مقادیر متفاوتی از شیوع کلی سندرم پاهای بی­قرار در مطالعات مختلف ذکر شده و عوامل مرتبط با آن در جامعۀ ایرانی مشخص نیست. بنابراین هدف این مطالعه بررسی شیوع و عوامل مرتبط با سندرم پاهای بی­قرار در ایران به روش متاآنالیز انجام شده است.
مواد و روش­ها: پژوهش حاضر بر اساس چک لیست PRISMA انجام شد. مستندات به دست آمده از بانک­های اطلاعاتی فارسی و انگلیسی زبان از جمله Magiran, Sid, Medilib, Iranmedex, Irandoc, Civilica, ISC, Web of science, Pubmed, Scopus, Springer  و موتور جستجوگر Google Scholar با محدودیت زمانی 1 جولای 2014 تا 11 نوامبر 2019 همراه بود. جستجو و استخراج داده­ها توسط دو نفر از پژوهشگران به صورت مستقل انجام شده و تحت یک ناظر سوم به صورت گروهی تصمیم گرفته شد. تمام مقالات مرور شده دارای معیار انتخاب، با چک لیست STROBE مورد ارزیابی کیفی قرار گرفته شدند. برای تجزیه و تحلیل داده­ها نیز از مدل اثرات تصادفی به روش متاآنالیز توسط نرم­افزار CMA-2 استفاده شد.  
یافته ­ها: در 24 مطالعه با 26474 نمونۀ ایرانی که 65.9% از آنان زنان بودند، شیوع سندرم پاهای بی­قرار 32.9 درصد (فاصله اطمینان 95 درصد: 44.7 -22.9) برآورد گردید. شیوع این اختلال در زنان ایرانی 47.6 درصد (فاصله اطمینان 95 درصد: 67.2 -28.7) و در مردان ایرانی 7.36 درصد (فاصله اطمینان 95 درصد: 55.3 -21.4) بود. بیشترین و کمترین شیوع سندرم پاهای بی­قرار بر حسب مناطق جغرافیایی به ترتیب در شرق کشور (44.3 درصد) و در شمال کشور (15.5 درصد) بود که این اختلاف بر اساس تحلیل فرارگرسیون معنادار بود (0.05>p). میزان شیوع این سندرم به ترتیب در بیماران همودیالیز 50%، سایر بیماری­ها 35%، زنان حامله 25.7%، سالمندان 20.2% و بزرگسالان 13.8% بود.
نتیجه­ گیری: شیوع سندرم پاهای بی­قرار در ایران بالا می­باشد که در مقایسه با مطالعات مشابه انجام شده در سایر نقاط دنیا بیشتر است. این موضوع توجه به تشخیص زودرس، پیشگیری و درمان این اختلال را در بزرگسالان و به خصوص زنان الزام می­دارد.
