en بررسی نوع و میزان خطاهای ثبت پرونده پزشکی در بیمارستان آیت ا... طالقانی تهران بهداشت محیط Environmental Health پژوهشي Research <div style="text-align: justify;"><br> <span style="font-family:b lotus;"><span style="font-size:12.0pt;"><strong>مقدمه</strong>:</span></span> <span style="font-family:b lotus;"><span style="font-size:12.0pt;">ثبت اطلاعات پزشکی در پرونده های بیمارستانی در حقیقت مستند سازی فعالیت های تیم پزشکی در بیمارستان است. بنابراین ثبت صحیح، کامل و به موقع اطلاعات بیماران می تواند نقشی اساسی در بهبود فعالیت های آموزشی، درمانی، تحقیقاتی، قانونی و آماری ایفاء نماید. مطالعه حاضر با هدف بررسی نوع و میزان خطاهای ثبت پرونده پزشکی و عوامل موثر بر آن در بیمارستان آیت ا... طالقانی به انجام رسید.</span></span><br> <strong><span style="font-family:b lotus;"><span style="font-size:12.0pt;">روش ها</span></span></strong><span style="font-family:b lotus;"><span style="font-size:12.0pt;">: مطالعه حاضر از نوع توصیفی- تحلیلی به شیوه مقطعی انجام شد. نمونه ای به میزان 330 پرونده بیماران در بیمارستان آیت ا... طالقانی از طریق چک لیستی خودساخته بررسی شدند و داده ها با نرم افزار </span></span><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:12.0pt;">SPSS</span></span></span><span style="font-family:b lotus;"><span style="font-size:12.0pt;"> و روشهای آماری توصیفی و آماری تحلیلی تجزیه و تحلیل شد.</span></span><br> <strong><span style="font-family:b lotus;"><span style="font-size:12.0pt;">یافته ها</span></span></strong><span style="font-family:b lotus;"><span style="font-size:12.0pt;">:</span></span> <span style="font-family:b lotus;"><span style="font-size:12.0pt;">بررسی میزان خطاهای پرونده ها نشان می دهد در بین خطاهای مورد بررسی، عدم ثبت تشخیص و عدم ثبت زمان مصرف دارو در بیش از 50 درصد پرونده های مورد بررسی بصورت دقیق ثبت نشده بود. کمترین خطا نیز مربوط به نداشتن ساعت و مهر بود. رابطه معنی داری بین خطاهای ثبت پرونده و برخی خصوصیات دموگرافیک مشاهده شد.</span></span><br> <strong><span style="font-family:b lotus;"><span style="font-size:12.0pt;">&nbsp;نتیجه گیری</span></span></strong><span style="font-family:b lotus;"><span style="font-size:12.0pt;">: با توجه به نتایج و وجود خطا در ثبت پرونده ها، اهتمام پزشکان و پرستاران بیمارستان در ارتقاء مستندسازی پرونده ها ضروری بوده و برای این منظور، می توان از راهکارهایی نظیر، آموزش اولیه برای دستیاران تازه وارد، روش های تشویقی و ارزیابی دوره ای پرونده ها بهره جست.</span></span></div> <strong>Introduction:</strong> Recording Medical information in hospital records are in fact documentation of the medical team activities in the hospital. Therefore, correct, accurate, and timely record of patients&#39; information can play a vital role in improving educational, medical, research, legal, and statistical activities. This study aimed to investigate the type and amount of errors in medical records documentation and its effective factors in Ayatollah Taleghani Hospital.<br> <strong>Methods:</strong> This descriptive-analytic study was cross-sectional. A sample of 330 patients&#39; records in Ayatollah Taleghani Hospital was investigated through a self-made checklist. Data were analyzed by the SPSS software and descriptive and analytical methods.<br> <strong>Results:</strong> The amount of error in the records showed that, among the examined errors, the lack of record in diagnosis and drug use time in more than 50% of the cases were not accurately recorded. The least error was due to the absence of time and stamp. There was a significant relationship between medical record errors and some demographic characteristics.<br> <strong>Conclusion:</strong> According to the results and the existence of errors in recording files, hospital doctors and nurses&#39; efforts to promote the documentation of cases were necessary.
